NEW PATIENTS:

Fill out the call-back form below or call our clinic at (905) 631-5353. Our Patient Care Coordinator will call you to discuss our patient onboarding process, discuss our clinic policies, and schedule you for your appointment. We will call you back as soon as possible however it may take a few days to hear from our staff as we are a high-volume clinic.
